You can pick it up at CIT Smitsborg (Zernike Campus) or we can send it through internal mail.

In order to create your official UTQ certificate (digital and hard copy version), we need the following information:

Initials

Full name
First, (middle) and surname(s)

Official title(s)

Date of birth

Place of birth & Country of birth
If the place of birth is outside the Netherlands, please specifiy the country.

Certificate Language
Would you like to receive the UTQ certificate in Dutch or English?

I give permission to use my personal records to create the certificate. After each UTQ ceremony, the old data are deleted from our systems.
